Custom Tattoo Booking Form
Please read my FAQ before filling out a form!
All clients must be 18+ or older - no exceptions.
By filling out this form, you are allowing me to get a thorough understanding of the tattoo you are looking for. Please fill out this form as detailed and concisely as you can.
I do not book customs ‘first-come-first-serve’. After keeping this booking form open to submissions for 24-hours, I review each submission carefully and with much consideration. I will announce when my books have closed, and will follow-up within the following few days.
Please know that submitting a booking form does not guarantee that I will be able to take on your submitted project.
After reviewing your submission, I will decide if I feel like I am the right artist for what you’re asking. My decision is not a personal reflection of you, but rather to make sure I am comfortable and feel capable of completing the submitted idea.
If after reviewing your custom submission I feel like I am a good fit for the work you are seeking, I will reach out via email to talk budget and scheduling. To book a custom, you will be asked to pay a $100 deposit. In addition to holding your spot, this also acts as a ‘drawing fee’.
PLEASE NOTE: I take appointments Tuesdays-Fridays during the day (generally 10am-2:30pm) — I do not book on weekends! If this doesn’t work with your schedule, please be respectful and don’t ask me to work outside my set hours.
