Custom Booking Form

All clients must be 18+ or older - no exceptions.
By filling out this form, you are allowing me to get a thorough understanding of the tattoo you are looking for. Please fill out this form as detailed and concisely as you can. All estimated budgets for projects will be for machine-tattoo work. I am not taking on handpoked projects at this time.
I do not book ‘first-come-first-serve’. After keeping this booking form open to submissions for 24-hours, I review each submission carefully and with much consideration. I will follow-up with the projects I choose to take on within the following days. Please be patient!
Please know that submitting a booking form does not guarantee that I will be able to take on your submitted project.
After reviewing your submission, I will decide if I feel like I am the right artist for what you’re asking. My decision is a reflection of how comfortable and capable I feel about executing your submitted idea. I will reach out via email to discuss budget and scheduling further.
If selected, you will be required to pay a $100 non-refundable deposit before scheduling. This deposit will go toward the total cost of your tattoo, provided you do not cancel or no-show. This also covers the ‘drawing fee’ required in custom work.
All customs projects start at $200.
