Custom Booking Form

All clients must be 18 or older — no exceptions.
This form helps me understand your tattoo idea — please fill it out clearly and concisely. All budget estimates apply to machine-tattoo work as I am not accepting handpoke projects at this time.
I do not book on a first-come basis; I review every submission carefully and will follow up with the projects I choose to take on. Please be patient with me! Submitting this form does not guarantee a booking with me.
After reviewing your idea, I’ll decide if I’m the right fit based on my comfort and ability to execute the design. If selected, I’ll email you to discuss budget and scheduling.
A $100 non-refundable deposit is required to schedule. It goes toward the final cost of your tattoo and includes the drawing fee for custom work, as long as you do not cancel or no-show.
All customs projects start at $200.
